GENERAL POLICIES
 
   

To ensure consistent communication with all patients and to help maintain a long-standing practice in the community, we would like to outline our policy toward the payment for services.
   
 
  • Patient co-payments are expected at the time of the appointment. If a patient is not aware of his or her co-pay amount, the average co-pay of $25 will be collected at the time of service. If adjustments are to be made, the company will do so proptly after the Explanation of Benefits is received from the third party payer.
  • Payment on an established account is due within 30 days by the insurance or responsible party (patient, parent or legal guardian). Although we are billing these insurance companies as a courtesy to you, you are personally responsible for your account.
  • If the problem for which you are seeing the physician involves litigation, such as may result from an automobile or work-related accident, be advised that we do not wait for payment until litigation is settled.
  • Your signature authorizes us to contact references in the event it becomes necessary to locate you.
  • You are encouraged to contact the Clinic Administrator if you have any questions regarding your account. Past due accounts are required to pay cash at the time of service.
  • All credit balances may be held as credits against future services rendered, unless credit refunds are requested in writing by the patient and patient care has been deemed complete by the physician.
  • A one-time processing charge of $25 and a 1% per month (12% per year finance charge) may be imposed on all accounts 60 days past due.
  • A $25 fee will be charged on all checks returned to the bank for lack of funds and are subject to Oregon Law, ORS #30.700, which states legal action can be taken for three (3) times the amount of the check or $100, whichever is greater.
  • In the event you do not give at least a 24 hour notice prior to canceling an appointment and/or if you do not show for a scheduled appointment or procedure, there will be a $25 administrative scheduling fee assessed. In the event of repeated "no shows," you may be asked to find another physician.
  • If your insurance deductible has not been met we require that a minimum of 50% of the allowable charges be paid prior to your procedure/surgery date. The charges for your office visit and procedure/surgery will be billed to your insurance company and any addition deductible and/or co-pay amounts will be due by you within 30 days of receipt of payment from your insurance provider. Any adjustment due will be made upon receipt of the Explanation of Benefits.
 
     
 
PAYMENTS PLAN
 
   

In an effort to provide payment options for our patients, the Northeast Oregon Surgical Clinic, LLC accepts the following forms of payment.

  • Cash
  • Checks
  • Debit Cards
  • All major credit cards (Visa, Mastercard, American Express, Discover)
  • Care Credit Payment Plans
  • Company sponsored payment plans*

 

Patient balances are due in full upon receipt of their statement. If you are unable to pay the balance in full by cash, check, or credit card, and if your balance due is greater than $300.00 we encourage you to apply for CareCredit as a possible payment option.

An annual interest rate of 12% and processing fee of $25.00 will be assessed to all accounts with a balance 60-days or more past due.

One option for you is CareCredit, an independent outside payment plan which works similar to a credit card but is designed especially for medical treatments. Upon credit approval, the benefits to you as the patient include: no Interest Payment Plans for up to 18 months, Extended Payment Plans for up to 60 months, and no processing fees. * Company Sponsored Payment Plans: If you do not qualify for CareCredit, another option is the company sponsored payment plan. 50% of anticipated procedure and office visit charges are due prior to the procedure date. The remaining 50% of the charges can be paid in 12 monthly installments at an annual interest rate of 12%, plus a processing fee of $25.00. If a monthly payment is not made within 7 days of the due date, your account will be sent to a collection agency.

If your procedure and office visits have already taken place, and you do not qualify for CareCredit, you may still take advantage of the company sponsored payment plan. 50% of the balance due must be paid at the signing of the financial agreement, the remaining 50% of the charges may be paid in 12 monthly installments at an annual interest rate of 12%, plus a processing fee of $25.00. If a monthly payment is not made within 7 days of the due date, your account will be sent to a collection agency.

   
     
 

CASH PAY PATIENTS

 
   

Dr. Andrew Bower and the Northeast Oregon Surgical Clinic are happy to offer care to Cash Pay Patients, otherwise known as patients who do not have insurance coverage and must pay for such expenses out of their own pocket.

Cash Pay Patients are required to pay for office visits in full at the time of service. When you check in with the receptionist, please indicate that you are a Cash Pay Patient and you will be given an estimate for the office visit charge. If you are not prepared to pay the estimate, your appointment will gladly be rescheduled for a later date.

Charges are determined by Dr. Bower and are based on the length of time the visit takes, the number of topics discussed during the visit, and the complexity of medical decision making required. At the time of check out, Dr. Bower will indicate to the staff how much your charges are and you will be required to make payment in full.

If Dr. Bower has recommended you undergo a surgery or procedure, payment arrangements must be made prior to being scheduled. Along with accepting Cash, Check, Debit Card, Visa, Mastercard, American Express and Discover cards, our office is proud to offer two payment plan options for our Cash Pay Patients:
